We didn't know we needed one until we bought the house and we were trying to figure out why the smoke was coming into the living room - fireplace is deep, so we couldn't initially see the hole in the fireplace floor. It was easy to just drop in. Solved the issue of the air displacement. Had bought a ash catch tray until it arrived to prevent any burning embers from going into the ash dump under the house. Great material, heavy duty. I have air tools at my disposal, so it was easy to modify but if you have no tools you will need to measure the opening precisely before purchasing.
