I found a website that said press and twist (and very faintly on the cap it does say that). Doing that made opening easy for me. I am not a vet so this is not medical advice. I found that Meric MOMETAMAX has the same 1% Clotrimazole, and the instructions for that say For 7.5 g, 15 g, and 30 g bottles: * Administer once daily for 7 days * 4 drops per ear for dogs < 30 lbs * 8 drops per ear for dogs = or > 30 lbs My thinking is 1 drop 4/8 times a day is much less likely to flow all the way down the ear canal to the eardrum. So I think it is better to do all the drops at once as this gives the best chance for the fluid to get all the way to the eardrum where the infection is. I did this for 7 days and my dog no longer is constantly flapping his ears.
