Q: Does this cap have the material all around? Some have nylon for the back panels?

Q: Does this have velcro on the back as well?

Yes, the a velcro loop field on the back that is approximately 1" X 5".

Q: Med/large was too small, large/xl was slightly too big .. what are my options.. will it shrink a little in the wash?

It probably won’t shrink with washing as it is all synthic. Maybe the smaller size and thrown in the dryer would loosen up the fit for tou

Q: Is this cap low profile? I like my hats to come low to my ears. Will this hat do that?

So I'm not completely sure what you mean by low profile, but it comes right down to where the top of my ear meets my head perfectly
