Can this be used in a boring bar holder on a milling machine, or is it only for a lathe?

If you are asking if it can be used in a boring head the answer is no. As this is a right-hand boring bar and you would have to run the machine in reverse to get it to cut. If you ran it in reverse without some sort of mod to the boring head the head would unscrew from the shank as soon as you began to cut causing a spectacular crash. You need a left-hand bar. With that you can run in forward with no worries.

is this actually a 3/4" or a 20mm, i need a 3/4.

3/4'' is the size of the shank itself. I don't think it would be a mm bigger as most common lathe/mill tools are inch instead of mm
