How long do you leave it on before you rinse it off the directions I don't understand How long

You don't rinse it off at all. You put it on, it rubs into your skin much like a moisturizer, and by doing so, relieves the itching. Obviously, if you shower, you should reapply it to the area that itches. But no, you don't want to rinse it off, you would be defeating the purpose of an anti-itch cream.
