which insert is for flat feet

The cf440 inserts work well for flat feet. I have those in some of my shoes. The cf430 should work well also.

Small enough for ladies size 5?

I wear a size 9-1/2 but you really should take the online quiz or go to a store that has the Dr. Scholl's fit machine. They have worked wonders for me, I know longer have low back pain.

Do you take the shoes inserts out

I did take the shoe inserts out. These are not full length inserts though. And they are larger (thicker) than the inserts that came with your shoes. They do help with cushioning and support. Much more than the old ones. But they needed replacing after about 4 months.
