Q: Will this work without using the Piezoelectric pickup installed, or do you need to also install the Piezoelectric pickup as well?

It can work, but it is better to install piezoelectric pickups. It is not recommended not to install piezoelectric pickups

Q: Is there any cork or rubber padding inside the soundhole clip?.... It looks like just metal from the pics. Don’t want to scrape the top of the guitar.

The metal part is polished and smooth, so be careful when installing it will not scratch the guitar

Q: Do you also need to buy an amp?

As R.C. says, “you need to plug it into something”. That could be an audio interface or mixer to record through a DAW with your computer… or even into one of the inputs on an AKAI MPC to sample your acoustic playing. An amp is the most basic and least creative way to use it and imo the only way to get a really solid acoustic sound that has been electrified is through a very expensive acoustic amp and an equally expensive guitar that’s probably already got on board Acoustic Fishman pick ups.
