Does the adhesive side apply to the lens or the cab ?

Both. An adhesive side will seal the light housing, and the other adhesive side will sela to the surface of the cab.

Does this item fit the 2006 2500hd? it says so in the description but it said so for the 2003 1500hd as well and you said that this item does not

Gasket prevents leaks into the cab headliner.

Perfect fit, replaced the bulbs in my 3rd brake light and saw recommendations to replace the gasket due to leaks from the old gasket, the cutout for the fixture is in direct contact with the interior headliner.
