I would always size up when that close so 3-6.

Q: Is the head opening fitted enough around the neck so that it wont ride up and cover baby's face?

Yes, the neck opening is safe. My (small) 8-month old kicks up a storm in her sleep, and we do not have a problem with this sleep sack riding up and covering her face.
