Q: how much weight will it hold?

I have a heavy 3 tier metal fruit basket. 40 lbs fully loaded. Not worried about the weight. Doesn't come with screw. Make sure you get high strength screws the top screw is the stress point the hook itself is really strong.

Q: Does this have to go into a stud or can I hang without a stud ?

Depends on what you plan on hanging on it. If heavy, like a hanging plant, better go in a stud. If light, like a wind chime, probably ok with anchors.

Q: It is unclear what size this is. Your description says 12 inch but 10 inch is the only option in the selection. I need 12 inch.

This is Sandcastle Dreams, a seller of this item. I can't speak for other sellers, but the one I am selling measures 12" from wall to edge of hook, and is labeled as 12" on the tag. It's under the "10 inch" size because that's the only option that was available.
