Can I conceal carry on the small of my back, right hand draw?

Yes... But not sure how comfortable at would be. I was shown the best place to conceal carry and I have carried it this way ever since. Place the conceal holster on the inside of the waist band just behind your right hip bone. It will be easier to draw if needed. And it conceals really well. And is much more comfortable. I have carried a conceal carry guns every way to carry one. Different size of conceal carry guns as well. And I mostly carry behind my right hip bone. You will find your sweet spot, just by adjusting the gun and holster once behind the hip bone. Hope this helps...

Does this work with the manual safety p365?

Instead of cutting the Kydex, use a heat gun or a small propane torch (at a distance) and spot-heat the kydex just where the safety lever is. Kydex is a heat formed plastic and you can reshape it slightly to accommodate the safety with a little pressure with a tool (screwdriver or dowel) on the inside where the lever slides in. It hardens back up when it cools and you can re-adjust it 'til it is right.

Is there a owb model?

Not on this model but you can purchase a separate paddle attachment and convert it. I do this for most of my holsters and change them when appropriate.
