Does the leather protection pad interfere with opening the sd card slot on the sony a7iii? it sure looks like it would.

In my actual use, the leather protection pad will not interfere with the opening of the sd card slot. And the leather protection pad is very soft.

Can this be used with a smaller lens, on a z6ii?

I see no reason it would not work on the z6ii. As long as you have a eye hook on the camera, the ring will attach. This wrist strap works great for me. It's easy to attach.

Nice camera wrist strap.
