Q: The box says do not apply directly to skin...well how exactly am i supposed to use this?

No ice packs are recommended to be placed directly on the skin. You can either let it warm up a bit or use thermo-barrier, ie. paper towel, like you would with any ice pack. Some people I know like to keep them in the fridge so they aren't as cold. Hope this helps, Penguin Fingers

It’s made of water and and a non-toxic alcohol mixture.

Q: Can it be heated in a microwave for heat treatment?

A warm water bath works to warm it. Microwave could damage it.

Q: Has anyone used this to temporarily "shrink" a finger to help get a tight ring off?

I suppose it could help but I’d try butter or some type of oil after making your finger cold to take a tight ring off.
