Could i do an A4 sized image with this?

Yes just do it bit by bit. I've done A4 pictures on t-shirts, jumpers, bags and more and all come out great.

Would this be suitable for heating iron on patches for jackets?

I would imagine it’s fine, but always test the material before heating. I use it for vinyl on T-shirt and jumpers

Is rhis safe to use in the uk as says it has to be 110 - 120 v

Is it anything more than an iron?

I don’t have an iron so can’t really compare, it’s a completely smooth surface, with 3 different temp settings.
