When just using the flame setting and not the heater, does it run quitely?

No sound at all when just flame is running.

Has anyone used this extensively? I'm curious if it holds up. I don't care about the heater so much as the faux fireplace effect. :)

I got mine last winter and have been using it at least a few nights a week ever since without the heat and it still works fine. I did use the heat a few times last winter but mostly I just wanted the fireplace effect.

If I remove the flame panel, will the flames reflect on the back of my fireplace? Are the flames independent of the panel?

Yes it can be removed but the flame effect will not be as noticeable without it. If you have a black interior fireplace and the heater is against the back wall you will see the flames. They are generated by lights inside the heater independent of the panel but you just won't see it as well.
