What do the product dimensions refer to exactly? (20.5 x 13 x 3 cm).  Surely we need to know the maximum and minimum neck sizes for each size?

It's hard to see by measuring my large Gentle Leader. I have had both a medium and a large for our labrador, the first when she was an adolescent dog. To be honest the large is rather too big even though she is now 15 months. There is a lot of spare extra both under the chin and around the neck, but being adjustable it's fine and works!

is medium the correct size for retriever?

I think medium should be fine for a young retriever (mine is now 13 months and has been using it since she was 5 months). I also think medium would fit certain adults as there's plenty of adjustment length on the straps, but I can't be sure.....yet.

What size is best for 5 year old jack Russell and also what size lead

The small would be best for a Jack Russell. As far as leads go, I would suggest a small Halti lead - you need a double ended lead to get the full benefit of a Gentle Leader collar so you can gently steer your dog - I have 2 Old English Sheepdogs and Gentle Leaders have made walking a doddle!). The small and large Halti leads are the same length, but the small is narrower, so probably better with a smaller dog. Found these measurements online:  Small Halti Training Lead: 1.5 cm wide,  1 m to 1.7 m long. Large Halti Training Lead: 2.5 cm wide,  1 m to 1.7 m long.Hope this helps!

is this available with extra padding? a similar one seems to hurt my rottweiler

No, I can understand why it hurts as out of all the headcollars available I find gentle leader fits best. Maybe you are a size too small and need the next size?
