Q: I can't open it even though the arrows are lined up. How do you do it? I've tried unscrewing it and lifting it off and it doesn't budge either way.

Make sure the batteries are fully charged before use. Under normal conditions, press the power switch on the base after aligning the arrows to start the grinder. If it still can't start, you can contact us via Amazon.com email, our customer service representative will help you solve all the problems.

Q: Would this grind a small pill?

The instructions state that the grinder should only be used on leafy herbs and that grinding beans, seeds and nuts could damage it. They do make pill crushers and grinders, most are even less expensive. I would search Amazon for "pill grinder" and make a selection there.
