If this is used to cover live parts why does it have a big hole that careless fingers could be inserted?

The enclosure is suitable for circuit breakers or Amazon door bell transformer when fitted the cover plate whole is completely sealed. The enclosure also incorporate back plate fixings specifically for these components.

How wide is the hole in the front in mm. Not the unit itself as I’m looking for a din rail that will fit the Shelly pro 4m

Hole width = 35mm, hole depth = 46mm. It takes a standard 2-pole MCB / RCD (I run the earth past the unit inside the enclosure).

need to install an electric shower but no room in my consumer unit. Can i put a 40a breaker in this and wire back to the supply? What size breaker?

Yes you can do that. But as per new regulation better to use metal box.
