Q: I am 5’4” and 155 with larger thighs and small butt. What size to have calves a little loose?

I don’t think you will find the calves loose in these leggings. Not that they are overly tight. I’m 5’7” and 166 and the thighs are now tight on my size mediums with a comfortable calf fit. Hope that helps.

Q: Would these be comfortable after abdominal surgery?

Not sure, but they are loose enough, so it will not be pushing on your stomach
