Q: Is it guaranteed to not leech poison into my butter? Is there any lead used in the paint or glazing or whatever?

Sadly there have been a lot of quality assurance issues with items form China. The guarantee doesn't help if lead happens to be in there and it does leach into your butter. I mean, I guess they'd send you a new butter bell... you could just buy a lead test kit and test before using. Often it's difficult finding things that aren't from China these days. Hopefully that means they are being more diligent on regulations and inspections.

Q: What happen if I don’t add water in it?

If you don’t use water the butter will not go bad for a couple weeks or so (as long as it is not contaminated with food, of course), but it will also not be as fresh tasting. The water serves as an airtight seal to keep the butter from developing a rancid taste.
