Q: On both sides there are two plastic caps covering the assembly and every time when I use it, they are falling off. How do I fix it?

Indeed, I find these plastic caps an annoyance for the same reason. In exasperation, I just left them be - uncapped. If there be any purpose at all, it is merely to cover the two short posts which support the device's piston assembly (that which moves the spiral up & down). To date, my uncapped bottle opener still functions well sans caps.

Loosen the spiral cap on top, change the spiral and tighten the spiral cap afterward.

Q: does the gold color fade?

No it hasn't faded. Totally worth it. Makes opening a wine bottle so easy

Q: It worked well at first but now refuses to reset and twist the screw out of the cork without pulling it out, what can I do?

These are only guesses as I have not had this issue. First make sure that you use the full swing of the handle. Second make sure the corkscrew is fully seated. I believe they screw in so that they can be replaced. Hope that helps. I like mine more than my original review as I no longer have to run the screw completely through the cork.
