Q: Does this work well on carpet?

This is not good on carpet. I am 160lbs on short berber carpet. After 18 months of use, the mat is full of cracks and chipped out holes as to be useless.

Q: Does it ship flat in the box or roll up?

It does ship rolled up. Comes in a narrow box and will need to be flattened. I recommend placing heavy flat items on the corners for a few days.
