Q: Do the legs/feet have a protective bottom to keep it from moving and scratching hard wood floors?

I put rubber caps on them to prevent scraping on Floors

Q: does it sag in the middle upon opening? Not sure if I got a used one or what

Mine didn't necessarily sag, but it's not as taut as others I bought in the past. Once there is dog weight on the cot it seems to lay more tautly.

Q: What is the maximum weight restriction for pet?

We have two pets pigs that weigh over 150 pounds and this was able to hold them too.

Q: Does it lock into place?

Not sure of the question as far as locking in place. This is much like a folding camp chair. It's built very well supports a dog or other pet with no problem. I highly recommend this. Our Yorkie which was 18 lb. Yes a big Yorkie. Took a little bit to get accustomed to it but at this point he loves it and looks forward to sitting in it. Highly recommend!
