I'm looking for an insole to provide extra cushioning for the ball of my foot area. Will these help with that?

I would give these a try. These are the most comfortable and durable insoles (in my opinion) in the market. I have been using them for many years and they are very comfortable. I wear cowboy boots everyday and are on my feet most of the day. I always have these in my boots. I find that they provide cushion to the entire foot.

Can Spenco Rx Comfort Soles be TRIMMED to the correct size? I'm a 6 1/2 so I need a size 7-8?

I wear a 11 1/2 running shoe and the 10-11 is too small. I am ordering the 12-13 and will trim as needed. I have trimmed them before and they were great. But try to get a smooth cut around the edge.

Do they have to go in between footbed and insole or can they go on top of insole?

They can go on top of the insole. Whatever is more comfortable.

Does anyone know how these compare with the new athlete insert from Dr. Scholl

Are you referring to the Dr. Scholl's Active Series Insoles? While I am not all that familiar with the Active Series insoles, the differences between these and the Spenco Comfort Replacement insoles are apparent. The Spenco Comfort insoles are a thin (5/32" thick), flat cushioned insole with no arch support made of the SpenCore material, whereas the Dr. Scholl's Active insoles are "designed to support your foot while in motion." The Dr. Scholl's Active insoles offer a deep heel cup and arch support, and they appear to be a bit thicker than the Spenco Comfort insoles. I wouldn't say that one insole is better than the other, as they are both quite different from one another - the insole that will work for you would really depend on the type of insole you are looking for (flat and cushioned, or supportive and cushioned) and the type of shoes you will be using the insoles with.
