Yes, it's an extra head for the figure.

Is the figure flexible?

Not sure. Was a gift I purchased.

Why are they 250$? Are the metal or something?

Definitely not worth it. Very small action figure with pieces that easily fall off and get lost. I assume the price increase which is about 10x when it was retail about 3 years ago is because it has been discontinued and this is now a rare find.
