Not sure chest size but I wear a XXL and it fits fine and it’s fully adjustable.

Are there pockets on the back side to hold other things such as notepads??

No, just little slots for pens, flashlight, etc...I'd always stick a small notepad in the other radio slot when I wasn't in need of a 2nd radio.

Is this product big guy friendly? I'm a 4x uniform shirt
